HCSF is designed to improve a patient's quality of life and help to alleviate their family’s stress while providing efficient loving care, eliminating pain, and easing overall emotional, spiritual, social and economic worries that comes from dealing with impending death. The program's interdisciplinary team of professionals and volunteers work closely with the family during the illness, and through the bereavement period which could be as long as a year.
